Life Threatening Allergies
The district must be notified when a student has been diagnosed with a life-threatening food allergy or another life-threatening allergy.
Parents are required to complete and provide at the beginning of each school year:
- The Anaphylactic Allergy Action Plan, completed and signed by the child’s physician and parent each school year along with their prescribed emergency medication
- The Authorization for Self-Carry and Self-Administration of Anaphylaxis Medication (EpiPen) must be indicated by a physician on Food Allergy Action Plan, if applicable
If the parent is requesting meal substitutions or modifications, the Special Dietary Accommodations Request Form from the Food and Nutrition Services (FNS) is completed by the child’s physician and parent.
