Special Diet Requests

Any diet modifications must be requested by a recognized medical authority (licensed doctor/physician, nurse practitioner, physician assistant). If your child requires a special diet, please have your licensed physician complete the required Dickinson ISD Food and Nutrition Services Dietary Special Request Form, available for download from our website.  No diet modifications will be made until the physician completed form is returned to Dickinson ISD Food and Nutrition Services.  If we do not receive a completed Dickinson ISD Allergy Form from a recognized medical authority (licensed doctor/physician, nurse practitioner, physician assistant), your child will receive a regular meal tray.  Medical statements completed by parents or guardians will not be accepted. It is preferred that an updated form be completed by the physician and turned into Dickinson ISD Food and Nutrition Services annually.

Allergen Information

Nutrition and Allergen information posted is based on information that Dickinson ISD Food and Nutrition Services currently has on file. Nutrition and Allergen information is subject to change based on changes from manufacturers.  Product reformulation may occur at any time and may not be known by the Food and Nutrition Services Department.  Dickinson ISD is not always notified of these changes.  In addition, substitute products may be delivered due to product availability.

Nutrition and Allergen information posted is for parent and student information.  Dickinson ISD Food and Nutrition Services does not monitor any a la carte purchase for allergens for any student. 

Please contact the health care provider on your child’s campus regarding any known food allergy or as soon as possible after any diagnosis of a food allergy.

The District has developed and annually reviews a food allergy management plan, which addresses employee training, dealing with common food allergens, and specific strategies for dealing with students diagnosed with severe food allergies.
